Submit Documents
To submit the documents for your adaptive Sailing and SCUBA Diving journeys with Sea MD, you have three options:
1. Click the button "Submit via Email" found on each page of the document. (Submit only once.)
2. Scan and email the documents to info@seamd.org
3. Alternatively, you can make copies of the documents for your records and send them via mail to:

Participant Application
Who needs to submit a Sea MD Participant Application?
- Sea MD Participants who have never submitted an application to Sea MD.
This document gives us contact information and personal details about the participant that we need to provide the best possible care for the participant while on a Sea MD adaptive Sailing and SCUBA Diving journey.
There are four optional addendums, which provide us with details about specific issues pertaining to the participant. Please include all addendums that apply to the participant.
It is essential that you fill out the application and all applicable addendums completely and accurately so we can ensure a safe and enjoyable journey for the participant. After we receive your application, if we require additional information, a Sea MD team member will contact you with specific follow-up questions that must be answered before your application is considered complete.
We keep Participant Applications on file for five years before we require a new application. If the condition of a participant changes within that time, contact us via phone or email, and we will update our information. If an address or contact information changes, you can re-submit the first two pages of the application to us separately. Please contact us to obtain an electronic PDF form containing just the first two pages in this event. You can download the full application with all addendums (just skip the addendums that do not apply) or download the main application by itself. If at a later time you need to complete an addendum separately, those files are available upon request.
Participant Agreement & Release Document
Who needs to submit the Sea MD Participant Agreement & Release?
- Participants who do not have a guardian MUST sign the Participant Agreement & Release.
- If the participant has a guardian, both participant and guardian MUST sign the Participant Agreement & Release.
This is a required document that details the terms and conditions of our agreement to include the participant on one of our adaptive Sailing and SCUBA Diving journeys.
Journey Signup Form
Who needs to submit a Sea MD Journey Sign-up Form?
Participants who are signing up for a Sea MD Journey. Use this document to sign up for a specific trip. We use the Journey Sign-up Form to schedule the participant on the requested Sea MD journey and plan for the specific personal care required.
Submit the completed form to Sea MD before the journey sign-up deadline, which is posted along with each journey description. If you are sending a check by mail to the address above, you can also include this form. If you are mailing in the form and check a few days before the journey deadline, we recommend calling or also submitting the Journey Sign-up form via email so that we can reserve a spot for the participant.
