What if we need funding or sponsorship?

You can still start the process. Complete the Journey Sign-Up form, then visit Funding & Grants or contact us to coordinate with a clinic, nonprofit, foundation, or benefits administrator.

Where do I find required forms and releases?

Use the Forms & Releases page. If you have questions about a specific form, contact us and we’ll help.

Does submitting the form guarantee a spot?

Submitting the form starts the registration process. Final confirmation depends on schedule capacity, support requirements, and completion of required paperwork.

What if a clinic or organization is referring us?

Ask them to contact us through the Contact page and select a partner inquiry. We’ll coordinate documentation and next steps.
